Details

Dr. Mitsuo Tomita will be joined by Valerie Markarian, Business Development Specialist, Sharp HospiceCare. They will discuss what is hospice, who qualifies for hospice, who makes that determination, what is covered by hospice, what is not covered by hospice. Details regarding the value of advance care planning and advance directives and POLST will also be discussed. An advance directive is a document done in advance to express what you want and don’t want done should you become incapacitated and unable to express your consent or refusal for medical treatments. POLST is a document signed and completed by a physician or nurse practitioner or physician's assistant that specifies if you do want or don't want cardiopulmonary resuscitation attempted if you go into cardiac arrest outside the hospital, if you do want or don't want tube feeding, etc.
